Objective: to understand the experiences of a group of female surgeons regarding the challenges and strategies to increase gender equity in the field. Method: qualitative, exploratory, ethnographic research, conducted from February to November 2023, using snowball sampling, via Google Forms®. Participants were active female surgeons living in the state of São Paulo. A semi-structured interview guide was used to explore the professionals' sociodemographic aspects and experiences. Content Analysis was used. Results: the 40- to 49-year-old age group predominated, with low ethnic and racial diversity. There were recurring reports of moral and sexual harassment and a constant need to reaffirm competence. Women reported being discouraged from choosing surgical specialties from graduation onward and suffering when opting for motherhood. Final considerations: research on female surgeons carried out in Brazil does not address the voices of these women. Their stories are important for changing the scenario of discrimination. Participants emphasize the need for mandatory institutional training on gender, effective reporting mechanisms, and greater support for family planning. Female participation in surgical specialties is limited, requiring institutional initiatives to create a more equitable and inclusive environment.
